For various reasons this has led to some fake OSHA cards being produced. This is not breaking news in any way. We have all seen the news reports of fake OSHA cards being produced not only in New York City but everywhere. This is one reason for the new design of the OSHA outreach training cards in Construction, General Industry, Maritime, and Disaster Site Worker. It was helpful to add the QR code to help validate the training. Here is a question to ponder though: If someone can produce a fake card with QR code, don't you think that they can produce a fake website to communicate a fake positive validation of the card? I have heard this to be true. What do you do then? You go to the source. With the potential of being fined upwards of $5,000.00 in NYC for having someone working on site without an OSHA card or a fake one, it would be worth the time to be sure. And this can be curtailed by the NYC Chapter 33 Building Code 3321.2 requirement to fill out daily training logs. I have noted below some of the websites that are the "source" to validate OSHA cards you may come across:

For those who are going for an OSHA training in person, here is an important tip: ASK to see their "OSHA Authorized Trainer" card. This will potentially save you from a bad situation. I recently went for my OSHA 502 so that I could renew my status as an OSHA Authorized Construction Trainer. They told us that we need to show this card at the beginning of teaching the OSHA 10 or 30 class. So I would recommend that you don't continue in the class unless you see this card from the instructor. I could imagine the kick in the gut feeling that you would feel if you just paid your hard earned money for a class and don't receive a real OSHA 10 or 30 card. Another thing that you can do before you organize or attend a class is go to the OSHA.gov website to see a list of trainers for your area. This will give you the peace of mind that your are talking to a real authorized outreach trainer. OSHA's website also gives you a list of the limited number of online companies that do provide the OSHA outreach training. Be sure to use one of them if you do go that route.
New York State Inspector General Lucy Lang today announced that an investigation of a construction worker found using a fake OSHA card on a major state infrastructure project in Manhattan has led to the arrest of two individuals who allegedly engaged in a widespread scheme to distribute fake identification documents.

"Workers on major construction projects are required to have proper training and OSHA certification in order to ensure worksites are safe for everyone," Inspector General Lucy Lang said, announcing the charges on Monday. "Our office uncovered a scheme to provide fake credentials to circumvent these critical safeguards, which led to a widespread fake ID operation. Thanks to the work of our office and multiple partners, we have shut down this fraudulent act. Meanwhile, we will continue to provide critical oversight of state infrastructure projects to identify and resolve any issues that may jeopardize safety and integrity."
OSHA 10-hour training is designed for entry-level workers in construction and general industry. While the OSHA Outreach Training Program is voluntary, employers may require you to earn an OSHA 10 card before you start work. This training is also mandatory in certain states, cities and local jurisdictions.
When you successfully complete OSHA 10-hour training, you will receive an official OSHA 10 DOL card in construction or general industry. We'll deliver your plastic, wallet-sized OSHA 10 card by mail free of charge.
Immediately after you finish the course, you can download a certificate of completion. While you wait for your OSHA 10 DOL card to arrive in the mail, you can provide the certificate to your employer to prove you completed your training.
Acquiring an OSHA 10 DOL card proves to employers that you have completed 10 hours of OSHA-authorized training on critical workplace safety topics. Entry-level workers with this credential have industry-specific knowledge and skills that help prevent injuries and keep workplaces safe and productive.
Your OSHA 10 card will never expire, but your employer may require you to retake the course every three to five years to refresh your knowledge and stay up-to-date with OSHA safety standards. If your OSHA card was issued more than five years ago, some employers may not accept it.

a) Replacing Lost or Damaged Student Course Completion Cards. A student that lost or damaged their student course completion card must contact their Outreach Training Program trainer to request a replacement student course completion card. The Outreach Training Program trainer must then contact their Authorizing Training Organization for a replacement student course completion card.
Since OSHA Outreach courses come with a durable plastic wallet card upon completion, issued by the Department of Labor (DOL), a completed OSHA 10 and OSHA 30 course is sometimes referred to as a "DOL card."
Private employers in high-risk industries sometimes require an OSHA 30 card, either as a hiring requirement or as part of their worker training program. Many federal contractors require an OSHA 30 card due to the heightened safety training requirements on military and other government projects.
Philadelphia requires all licensed contractors to employ at least one supervisory worker with a recent OSHA 30 Construction card (and on certain projects, such a person needs to provide continuous oversight of the work). Plus, workers need OSHA 10.
Since OSHA 30 expiration depends on jurisdiction and employer (and either one can change their requirement at any time), it would be nearly impossible for the DOL to put an accurate expiration on your wallet card.
Instead of certifying workers, OSHA offers voluntary outreach training programs and authorizes trainers or training providers to deliver the 10-hour or 30-hour courses. Finishing the industry-specific program leads to the issuance of an official completion card from the comfort of your home.
While the completion card does not expire, it is recommended to take OSHA outreach safety training every 4 to 5 years to stay updated with the latest safety regulations and industry practices. For complete training guidelines, workers are recommended to consult with local regulatory authorities.
Have you lost or damaged your 10-hour or 30-hour card? Contact your OSHA-authorized outreach training provider to obtain a replacement card. It is important to note, however, that the replacement card can only be issued to students who have taken the outreach program within the last five years. Only one replacement card can be given per student.
So make sure to request a replacement card before the 5th anniversary of your course completion date. What if you had taken the course more than 5 years ago? OSHA-DOL will not issue a replacement card. To receive a new and valid course completion card, you will have to take the corresponding OSHA outreach training program again.
Whencreating your own certificates and/or wallet cards, you should use your companyname and/or logo, contact information, etc. You should include the location andthe date of the training and the name of the person authorized to deliver thetraining.

New entrants to the construction or demolition workforce may begin working if they acquire a Temporary Site Safety Training Card (a non-renewable card that expires after 6 months) by completing an OSHA 10 Hour Class.
